What is Care Management?

Certified Care Managers coordinate resources to meet the immediate and long-term care needs of your loved ones facing challenges with aging or disabilities.

Don’t navigate the healthcare maze alone.

Patricia Shelton, founder of Specialized Care Management, Inc., experienced first-hand the exhaustion of managing the overwhelming care needs of her aging parents. Her journey inspired the creation of a senior care concierge service to support others struggling to manage the health and well-being of their aging or disabled loved ones.

Our goal is to address the challenging needs of our seniors, those living with disabilities, and their families, managing their immediate and long term care needs. We collaborate with home health agencies, the Veterans Administration, various health plans, financial and legal agencies, assisted living, skilled services, and specialty providers.

Ultimately, we bring families peace of mind as we improve healthcare outcomes and advocate for our clients’ overall well-being.

Your Northern Nevada team includes registered nurses (RNs) and professional, nationally Certified Care Managers (CCMs). With education and experience in a variety of fields – nursing, gerontology, social work, and psychology – our CCMs are uniquely prepared to coordinate all aspects of care for your loved one’s special needs or geriatric concerns.

Your aging life care expert serves as a guide and advocate to assist clients in attaining maximum functional potential in a suitable environment. Your CCM also coordinates care in a cost efficient manner, utilizing local resources and support systems based on the client’s needs and goals. Free from conflicts of interest, CCMs abide by a strict code of ethics and nationally reviewed standards of practice.

Start now and schedule your complimentary 30-minute telephone consultation. You’ll discover how an aging life care expert can meet your specific goals.

The first step is a Comprehensive Assessment conducted by a professional, nationally Certified Care Manager. This assessment is the foundation for identifying specific, practical, local solutions for your medical, safety, and social concerns.

Your personal Certified Care Manager can act as a liaison between you, your loved one, and all parties involved in providing or making decisions about care, including physicians and other healthcare providers, family members, caregivers, attorneys, and fiduciaries.

With ongoing monitoring and assistance to meet the agreed-upon goals of a comprehensive care plan, you’ll gain the peace of mind that comes with having a competent guide through the complex healthcare maze.
